Background
The Abbott FreeStyle Precision Neo is a Blood Glucose Monitor and Ketone Monitoring System manufactured by Abbott Diabetes Care Inc, launched in September 2014 in the US, following its approval in Europe in September 2013. Key features include individual packaging for its test strips and a memory that can store up to 1,000 readings. Both the meter and the test strips are available over-the-counter (without a prescription) at an affordable price.
It is a slim, user-friendly, self-testing meter designed for the monitoring of blood glucose levels and aid in management of diabetes. It has a large digital display for easy reading. It is intended for use outside the body (in vitro diagnostic use) for self-testing or professional use. The system may also aid in the management of diabetes by providing the user with suggested recommendations to insulin doses based on healthcare professional entered data.
The meter, lancing device and other accessories are for use by a single person. They must not be used on more than one person including other family members due to the risk of spreading infection. All parts of the meter, lancing device and other accessories are considered biohazardous and can potentially transmit infectious diseases, even after performing the cleaning procedure.
There had been reports in 2025 by US Food & Drug Administration's MAUDE database that the meter was producing erratic readings such as 398 mg/dl, 205mg/dl, 205 mg/dl, 37 mg/dl, and 180 mg/dl within 10 minutes. The most probable root causes associated with this failure mode are disconnected, faulty, or damaged components, software/data corruption, or misuse. Another MAUDE report in 2024 indicated a user experienced metabolic symptoms (dizziness, weakness) when the replacement meter delivery was delayed. The original meter had shown error codes (E1, E2, E3, E4) and was unable to be used for testing, highlighting the risk when device access is disrupted.
In July 2015, the device was subject to a Class II recall in Canada, due to a firmware defect affection the "basal titration" feature: where the meter could display 0 units instead of the basal insulin dose recommendation, potentially misleading users using long-acting basal insulin.
Identification
The FreeStyle Precision Neo is a compact, handheld medical device. It features a slim, lightweight body, a large digital display, and a five-second testing time for quick and easy readings. It is battery-powered lasting 3000 tests and is portable.
The serial number can be found in the Preferences Menu on the Home screen under About. Information about the device is displayed there, and further information on the device, continue to press "Info." It is also shown on a sticker on the back, bottom right corner, next to the icon SN, meaning, Serial Number.
