Why are the images always out of focus?
The camera used to always work fine, but now the images are out of focus, no matter how far away I am. What gives?

There is a small bead of adhesive that hold the lens at the factory set focal length. It is likely that this adhesive has broken free and the lens has moved. You will need to open the camera, adjust the lens to the desired focus, and re-adhere the lens in place.
