My phone is stuck in DFU mode, and when trying to restore via iTunes I receive error 21. The "hosts file" thing isn't applicable as I don't jailbreak (so the entries that you comment out aren't there), I've tried a different computer (3 computers total), and tried 2 different lightning cables all with no luck. Please help!

I tried two computers, same result. I took the iPhone 5 to an Apple Store. The phone was less than a year old, 9 months, and was replaced.
You have to go to an Apple store to get the replacement on site. When I went to AT&T first, they said they could send the phone in and wait for a replacement. Or, I could charge the replacement to my credit card and hope to be re-imbursed if the original phone was determined to be defective. At the Apple store it took 20 minutes for them to confirm the phone was defective and to replace it.
