Try lifting out and re-seating the card in its slot.

Try uninstalling and re-installing the driver for the card.

Note: You might try using DDU_Display Driver Uninstaller app to completely remove all traces of old, possibly corrupt driver.

If neither helps then replace card.

Might also want to check that both fans installed on the card are working.
