How to replace sm-n9208 (note5) rear camera glass lens cover

my note5 was normally laying on top of the table with cables underneath it, when it was accidentally pressed on by a heavy hand, it cracked. upon checking on the front lcd screen, there was no evident crack whatsoever. but when photo time came, it has a picture but the image was hazy. on checking at the back camera for smudges or fingerprint, there it was, a cracked glass lens cover. tried removing the glass shards, and checked the photo image again, now it was clear and crisp, but the auto focus is not working. found out in youtube videos that glass shards may be blocking the camera mechanisms auto focus movement, so make sure there are no residual glass shards around the camera lens. one this is for sure, since the glass shards were removed, it needs a lens cover replacement. how then, and what are the proper procedures on replacing the glass lens cover... making sure the autofocus is also functional...
