Can I replace the headphone jack?

I bought this phone a loooong time ago and the 3.5mm jack is going bad. I have to press it in a certain direction (Towards the board) so that it makes a connection and sends the audio up into the headphones. I was wondering if it were possible to replace the jack itself or if I have to buy another whole board for the phone. And where would I find either one of the parts at? Any help would be greatly appreciated!
