Why people can't hear me unless I use speaker?
I have iPhone 4S. I can hear people but they can't hear me talking unless I put it on speaker.

What's the history of that phone? When did this issue appear? Anything notable such as liquid damage, hard drop, previous repair?
If not, you'll need to walk through the troubleshooting process step by step:
When you use the speaker, you also use a front facing microphone.
When you talk with the phone to your ear, you use the bottom microphone.
That microphone could be clogged (see mesh opening in the bottom of the frame, left of the charging dock).
It could be defective, you'll need to replace the whole charging dock assembly. There's a guide for that on iFixit that also has links to the needed parts and tools.
If that still doesn't solve it, it's time to look at the logic board of the phone.
