How do ID my iphone 4 if the model number on the back cannot be red because the glass is shattered / missing?
I need to get replacement front glass / back glass.

Since the front glass is broken, it sounds like you'll be needing to replace the Display Assembly (as well as the Rear Glass Panel). The easiest way to tell which model of iPhone you have (given that you have fully disassembled it) is to look at the right side (looking from the front) of the Display Assembly.
In order of the stack:
iPhone 4s
iPhone 4 GSM
iPhone 4 CDMA

All of those screens are identical and it is only the plastic frame that differentiates them. It is held on with adhesive. You can buy a 4S LCD and put your iphone 4 frame on it and install it and it will work. The technology is identical. The back cover, on the other hand, has clips in different places which are not removable, so you need to get the right one.

that's for verizon but you need to be phone specific because at&t verizon have different backs.

GSM iPhone 4 has SIM Tray Slot and Antenna Band (black line) on top of iPhone. CDMA iPhone 4 does not have SIM Tray Slot. iPhone 4S has SIM Tray Slot and no Antenna Band (black line) on top of iPhone.
